Questions and Answers on Powers of Attorney: This Q&A is produced by Columbia Legal Services.  It may answer many of your questions related to Powers of Attorney.  It is not, however, a substitute for legal advice.

Financial Power of Attorney Forms

Durable Power of Attorney Form: This Power of Attorney form may be used if you want an "agent" to act on your behalf immediately.  It is "durable," meaning the "agent" will be able to act on your behalf even if you become incapacitated.

Power of Attorney Form - Effective Upon Disability ("Springing"): This Power of Attorney form may be used if you wish your designated "agent" to be able to act on your behalf only after you have become incapacitated.  A medical doctor must certify your incapacity before the document and your "agent's" powers of attorney become active.

Power of Attorney Revocation Form: This short form may be used if you wish to revoke a power of attorney document.  The revocation should be provided to your "agent" and any other institution or agency that has a copy of the document.  If your power of attorney document was recorded, provide a copy of the revocation to the recorder's office.  

Health Care Powers of Attorney and
Advance Directive (AKA "Living Will")

Health Care Power of Attorney and Advance Directive Form: This comprehensive packet is produced by Compassion and Choices of Washington.  It contains information about health care powers of attorney and advance directives along with a form in which you may express your wishes regarding health care should you become incapacitated and designate a health care "agent" who may make health care decisions of your behalf should you be unable to do so.  A large print version is available for download.
